If you are having trouble:
1) Turn off your Ad Blocker (pop ups must be allowed)
2) Clear your cache and cookies (may be under "Clear Browsing Data")
3) Restart browser and re-load this page

1. You will be asked for your user ID and password.
  • Your user ID is the first letter of your first name, followed by your full last name and the last four digits of your Social Security number. (Example: John Doe XXX-XX-1234 would be JDOE1234). *Use your name and SS#, not your spouse's.            
  • Please refer to your online instruction sheet for your password


2. You will be asked to change your password once you've logged in.
Please make sure your new password is 8-12 digits long and includes at least 1 capital letter and 1 number. Make sure it's something you will remember, and write it down.


3. Next you will be asked to make a security question.


4. Read the user agreement, then click "I agree".


5. Next you will see the "Registration Settings" page. Enter as much information as possible-anything with an asterisk is required. If there is information already there please verify that it is still accurate.


6. Complete the questionnaire, and be sure to answer all questions. There will be a "next" button at the bottom of each page that you will click to move on.


Please remember your results will only be as helpful and accurate as you make them, do not cheat yourself out of this resource. By taking your time and answering each question honestly you are ensuring yourself feedback that can have a large impact on your health. Your individual answers and personal information will not be shared with your employer.

Frequently Asked Questions

Q: Does my employer see my responses to the questionnaire?

A: No. None of your individual responses will be shared with your employer.


Q: What information is shared with my employer?

A: They will receive an aggregate report with population information such as "50% of respondents from your company reported that they always wear their seatbelt when riding in a vehicle." No information that can identify you with any response is shared.


Q: Do I log in using my spouse's information, since I am enrolled in the wellness program through their employer?

A: No. You will use your own information to access the questionnaire. Your user ID is the first letter of your first name, followed by your full last name and the last four digits of your Social Security number. (Example: John Doe XXX-XX-1234 would be JDOE1234). Please refer to  your online instruction sheet for your password.



Q: What if I have problems logging in?

A: Double check that you are entering the correct information for username and password. If you still have difficulties, you can call Wellness Partners at 1-877-345-7775 and let us know you are having trouble logging in or you can email processing directly. processing@wellness-partners.org.


Q: What do I do if I forget my password?

A: Click on "I forgot my password: reset it" on the bottom of your screen. Follow the instructions, if you did not previously provide an email .If you did not provide an email address, you may request a password reset by emailing processing@wellness-partners.org, or calling Wellness Partners at 1-877-345-7775.


Q: Do I have to complete the entire questionnaire in one sitting, or can I access it multiple times?

A: You may access your profile multiple times. All of your answers will save automatically, so they will be there when you return. However, it all must be completed within 7 days of logging in for the first time, otherwise you must start from the beginning. 


Q: Can I change an answer?

A: Yes. However, you can not move forward or backward through pages unless the page you are on is complete. Meaning, you must answer all questions on the current page and then click on "Previous" in order to access earlier pages and questions. You may also exit and log back in. This will show you all of your saved answers, and allow you to change them, starting from the beginning.



Q: When will I receive my report?

A: Once we have your lab results and completed Personal Wellness Questionnaire we can generate your report, which you will then receive within 2 weeks.


Q: Where is my report sent?

A:
Your report is sent to the address you provide at the beginning of the online Personal Wellness Questionnaire. It is very important to be certain you enter your address correctly for timely delivery of your report.
